Hospice Nurse Case Manager

Amedisys is a leading provider of home health, hospice and high-acuity care, dedicated to helping patients and families navigate the complex healthcare system. As a Hospice Nurse Case Manager, you will be part of a team of professionals committed to improving the lives of those they serve.
Responsibilities- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed or document rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness and according to care center policies.
- Record pain/symptom management changes/outcomes as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Regularly communicate with clinical manager and care team.
- Perform on-call responsibilities and on-call services to patients/families as assigned.
- One year of nursing experience.
- Current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation and liability insurance.
