Job Title: Hospice RN Case Manager

At Interim HealthCare Staffing, we are seeking a compassionate and skilled Hospice RN Case Manager to join our team. As a Hospice RN Case Manager, you will play a vital role in providing exceptional care to patients and families in need.

Job Summary:

The Hospice RN Case Manager will be responsible for maintaining a specified case management load, completing initial and ongoing assessments of patients' physical and psychosocial status, and developing and implementing plans of care in collaboration with the Interdisciplinary Group (IDG) and other healthcare providers.

Key Responsibilities:
  • Maintain a specified case management load, ensuring timely and effective care coordination.
  • Conduct initial and ongoing assessments of patients' physical and psychosocial status, identifying needs and developing plans to address them.
  • Collaborate with the IDG and other healthcare providers to develop and implement plans of care, ensuring seamless care coordination and patient-centered care.
  • Coordinate services provided to patients and families, ensuring timely and effective delivery of care.
  • Supervise and direct the services provided by LPNs, Hospice Aides, and Homemakers, ensuring high-quality care and patient satisfaction.
  • Evaluate and coordinate the need for services, including inpatient, respite, and continuous care, as indicated by patients' status and needs.
Requirements:
  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years, or evidence of current clinical knowledge as deemed appropriate by the DHCS.
  • One (1) year of hospice care experience, with a preference for precepting or training experience.
  • CPR certification.
  • Able to provide proof of valid driver's license and auto liability insurance.
