As a Hospice RN Case Manager, you will be responsible for providing exceptional hospice care to patients and families in Charleston, Berkeley, and Dorchester Counties.

Responsibilities:

* Maintain a specified case management load
* Complete initial and ongoing assessments of patients
* Develop and implement individualized plans of care
* Coordinate care among healthcare providers
* Direct and supervise LPN/LVN, Hospice Aide, and Homemaker services

Requirements:

* Graduate of an accredited school of nursing
* Licensed to practice as a RN in the state of South Carolina
* 12 months of accumulated experience as a registered nurse
* 1 year of hospice care experience
* 1 year of precepting or training experience preferred
* CPR certification
