Developmental Disabilities Coordinator

We are seeking a Developmental Disabilities Coordinator to join our team at ADAPT Community Network. In this role, you will be responsible for supporting individuals with developmental disabilities, ensuring they receive the necessary services and care to live fulfilling lives.
As a key member of our staff, you will develop and implement Individualized Service Plans that cater to the unique needs of each individual. You will work closely with clients, their families, and external agencies to ensure seamless coordination of services and support.
You will also be required to maintain accurate records and documentation, adhere to regulatory requirements and agency policies. Additionally, you will participate in interdisciplinary planning and provide training as needed.
Responsibilities:- Develop and implement Individualized Service Plans that meet the specific needs of each client.
- Collaborate with clients, families, and external agencies to ensure effective coordination of services and support.
- Maintain accurate records and documentation, adhering to regulatory requirements and agency policies.
- Participate in interdisciplinary planning and provide training as needed.
- Bachelor's Degree in Human Services or related field.
- At least one year of experience working with individuals with developmental disabilities.
- Excellent communication and interpersonal skills.
- Able to maintain confidentiality and handle sensitive information discreetly.
- Strong organizational and time management skills.
