Role Overview:
This role involves providing specialized care and support to patients with developmental disabilities. You will work closely with interdisciplinary teams to develop and implement personalized plans of care, focusing on maximizing each patient's potential and promoting independence.
Key Responsibilities:
- Conduct thorough assessments of patients' physical, emotional, and social needs
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
- Administer medications safely and accurately, adhering to established policies and procedures
- Document patient care and progress accurately and comprehensively
- Collaborate with colleagues to ensure seamless handovers and effective communication
