We are seeking a skilled and compassionate Registered Nurse to join our Hospice team at Prisma Health. As a Hospice Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals with terminal illnesses and their families.
Key Responsibilities- Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team.
- Provide symptom management and pain control, as well as emotional and spiritual support to patients and their families.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in bereavement support and counseling for patients and their families.
- Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
- Current RN licensure in the state of South Carolina
- 2 years of experience in hospice or palliative care (preferably)
- Valid driver's license and acceptable motor vehicle record
