Job Summary

Medical Services of America Hospice is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to our patients in the Columbia, Sumter, and Camden areas of South Carolina.

The ideal candidate will have a strong background in nursing and excellent communication skills. As a member of our hospice team, you will work closely with patients, families, and healthcare professionals to provide comprehensive care and support.

Key Responsibilities:

  • Provide skilled nursing care to patients in accordance with their individualized care plans.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination.
  • Conduct thorough assessments and develop effective care plans.
  • Communicate effectively with patients, families, and healthcare professionals.
  • Participate in on-call rotations to ensure 24/7 patient care.

Requirements:

  • Current licensure as an LPN in the state of South Carolina.
  • Minimum of one year of nursing experience.
  • Home health experience preferred.
  • Reliable transportation and company-required auto liability insurance.
