About the Role:

Gentiva Hospice is seeking a dedicated and compassionate Hospice Registered Nurse (RN) to join our team as a RN Case Manager. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in need of hospice services.

Responsibilities:

  • Provide direct patient care services, including evaluating patient and family needs, developing and implementing individualized plans of care, and collaborating with other healthcare professionals to ensure comprehensive care.
  • Act as a liaison between patients, families, and healthcare providers to ensure seamless communication and care coordination.
  • Manage patient clinical records, ensuring accurate and timely documentation.
  • Participate in quality assessment and performance improvement initiatives to enhance patient care and outcomes.
