Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Hospice Admissions Nurse. As a key member of our interdisciplinary team, you will play a vital role in coordinating the admissions process for hospice patients, ensuring their physical, psychosocial, and spiritual needs are met.

Responsibilities
  • Coordinates admissions of hospice patients by making skilled assessments of patients' initial physical, psychosocial, and spiritual needs.
  • Coordinates the establishment of the initial plan of care with the attending physician, medical director, and the interdisciplinary team.
  • Provides skilled interventions and instructions to patients and caregivers to provide optimum symptom control and patient comfort under the direction of the physician.
  • Functions as a liaison between disciplines and facilities during the admission process.
  • Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness, and according to policies.
  • Communicates significant findings, problems, changes in condition, or environment to the clinical manager/physician and other members of the hospice care team.
  • Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Performs other duties as assigned.
Qualifications
  • Current RN license, specific to the state(s) you are assigned to work.
  • One year of clinical RN experience.
  • Current CPR certification.
