We are currently looking for a dedicated Discharge Supervisor to join our team in a behavioral healthcare setting. This position plays a crucial role in the planning and execution of patient discharge processes, ensuring that all relevant parties, including family members and healthcare providers, are actively involved from the moment of admission.

Position Overview:
The Discharge Supervisor will oversee the comprehensive discharge planning for patients in Crisis, Inpatient, or Transition Point programs. This role includes participating in regular staffing meetings and daily huddles with a multidisciplinary team to identify and address any potential barriers to discharge. The supervisor will ensure that the discharge transition plan is accurately documented, reflecting all necessary details such as placement information, outgoing referrals, and follow-up appointments.

The Discharge Supervisor will collaborate closely with the Clinical Manager, Clinical Director, and Operations team to guarantee that discharge planning is both effective and efficient, meeting the needs of patients and the community while adhering to all relevant policies and regulations.

Qualifications:

  • Bachelor's Degree in a related field is required; Master's Degree is preferred.
  • 1-3 years of relevant experience in Behavioral Health Care is preferred but not mandatory.
  • Fingerprint clearance card is required.
  • Must possess a valid AZ Driving License.
  • Clear motor vehicle record for the past 39 months.


Compensation: $50,000.00 to $60,000.00 annually
