About the Role:

We are looking for a compassionate and organized Nurse with experience in dialysis care to coordinate patient care plans, collaborate with healthcare teams, and deliver exceptional patient outcomes.

Responsibilities:

  • Care coordination: Develop and implement individualized care plans for patients undergoing dialysis, ensuring seamless transitions between settings and services.
  • Patient education: Educate patients and their families on dialysis treatment options, self-care techniques, and disease management strategies.
  • Clinical support: Provide guidance and support to patients, caregivers, and healthcare professionals to optimize patient outcomes.
  • Data analysis: Track and analyze patient data to identify trends, opportunities for improvement, and areas for further education.

Requirements:

  • RN licensure in the state of practice
  • Minimum 1 year of RN experience, preferably in dialysis or related field
  • Excellent communication, organizational, and time-management skills
  • Ability to work collaboratively as part of an interdisciplinary team
