Responsibilities

  • Care Coordination: Collaborate with healthcare professionals, patients, and families to coordinate comprehensive care, ensuring timely and efficient delivery of services.
  • Documentation and Communication: Accurately document patient information, communicate effectively with stakeholders, and maintain accurate records to ensure continuity of care.
Requirements

To be successful in this role, you should possess:

  • A Bachelor's degree in Nursing, preferably with 1 year of experience in a related nursing specialty.
  • Current licensure as a Registered Nurse (RN) in the state of employment.
  • Ability to work in a fast-paced environment, prioritize tasks, and manage multiple responsibilities simultaneously.
  • Excellent communication, interpersonal, and problem-solving skills.
What We Offer

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, plus benefits such as medical, dental, and vision insurance, retirement savings plan, and paid time off.

This is a full-time position working Monday through Friday, 8-hour shifts.
