Hospice Chaplaincy Specialist

We invite a dedicated and compassionate Hospice Chaplain to join our community-based home health team. As a member of our interdisciplinary group, you will deliver exceptional patient care and contribute to our commitment to treating people like family.
Responsibilities:
- Facilitate spiritual assessments and develop individualized care plans.
- Deliver direct spiritual care services, supporting patients and families during challenging times.
- Maintain effective relationships with healthcare teams and community partners.
- Record services and participate in quality assurance activities.
Qualifications:
- Evidence of theological education or extensive experience in hospice care.
- Experience working with individuals, families, and caregivers.
- Excellent communication and empathy skills.
- Ability to navigate complex cultural contexts.
