Job Summary
We are seeking a skilled Hospice Care Specialist to join our team. This role involves providing personal care services to patients in their homes, including assistance with daily living activities, ambulation, and transfer. The successful candidate will have excellent communication skills, be able to work independently, and possess a strong commitment to delivering compassionate patient care.
Responsibilities:
- Provides personal care services to patients, including assistance with daily living activities, ambulation, and transfer
- Plans and prepares nutritious meals for patients, as assigned
- Performs homemaking and other environmental services, as assigned
- Observes, reports, and documents any changes in patient status
- Assists patients with prescribed exercises, as instructed by nurse or therapist
- Performs and records simple urine tests for sugar, acetone, and albumin
Requirements:
- High school diploma or equivalent
- Current state certification (or registration) as a nurse assistant, as per state certification/registration requirements
- CNA experience within the last two years
- Current CPR certification
