Job Summary

Springfield Medical Care Systems, Inc. is seeking a highly motivated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will play a critical role in helping individuals access the care they need through the provision of screening, brief interventions, and connections to services.

Key Responsibilities
  • Community Outreach: Develop and maintain relationships with community members, organizations, and healthcare providers to promote health and wellness services.
  • Screening and Brief Interventions: Conduct screenings and provide brief interventions to identify and address health and wellness needs.
  • Service Coordination: Connect individuals with community resources and healthcare services to address a range of needs, including clinical care, food insecurity, housing, and economic assistance.
  • Documentation and Reporting: Maintain accurate and timely records of patient encounters, referrals, and other relevant information.
  • Collaboration and Communication: Work closely with healthcare teams, community organizations, and other stakeholders to ensure seamless service delivery and coordination.
Requirements
  • Experience: 1+ years of experience in a healthcare or social service setting, preferably in a community outreach or patient advocacy role.
  • Education: High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
  • Skills: Excellent communication and interpersonal skills; ability to work independently and as part of a team; strong problem-solving and critical thinking skills; proficiency in computer-based tools and software.
  • Certifications: Valid driver's license and reliable transportation required.
