Job Summary

As a Community Health Worker, you will play a critical role in helping individuals access the care they need through the provision of screening, brief interventions, and connections to services.

Key Responsibilities
  • Community Outreach: Develop and maintain relationships with community members, organizations, and healthcare providers to promote health and wellness services.
  • Assessment and Referral: Conduct assessments to identify individuals' needs and provide referrals to appropriate healthcare services, including clinical care, food insecurity, housing, and economic assistance.
  • Case Management: Work with individuals to develop and implement care plans, coordinate access to services, and provide ongoing support and advocacy.
  • Collaboration and Communication: Collaborate with healthcare teams, community organizations, and other stakeholders to ensure seamless care coordination and communication.
  • Documentation and Reporting: Maintain accurate and timely records of patient interactions, assessments, and referrals, and provide regular reports to management.
Requirements
  • Experience: 1+ years of experience in a healthcare or social service setting, preferably in a community health or patient access role.
  • Education: High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
  • Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team, strong problem-solving and critical thinking skills, and proficiency in computer-based tools and databases.
  • Certifications: Valid driver's license and ability to travel to community locations as needed.
