Job Title: LPN Care Coordinator

Join our team as a LPN Care Coordinator at Advocate Aurora Health, where you will play a vital role in coordinating healthcare services for patients in the Advanced Care at Home (ACAH) program.

Key Responsibilities:
  • Care Coordination: Coordinate healthcare services for patients in the ACAH program, including DME, transportation, and other necessary services.
  • Clinical Inquiry Response: Respond to clinical inquiry calls regarding ACAH, triage as needed, and escalate potential risk management situations to supervisors.
  • Patient Virtual Monitoring: Assist with patient virtual monitoring as needed.
  • Communication: Practice and create an environment that supports and sustains effective communication with internal and external customers, including interfaces with Advanced Practice Clinicians (APCs) and other service providers.
  • Case Management: Provide basic case management to review symptom control with patients, prescription refills, scheduling/retrieving results on diagnostics, labs, consultant visits, and procedures.
  • Referral Process: Complete the referral process for Advocate Aurora Health departments as needed.
  • Data Entry: Ensure data entry into the system is complete for referrals, insurance requirements, and medical necessity support through researching medical history.
  • Clinical Resource: Act as a clinical resource to coordinate complex cases for safe and appropriate transition to other care settings.
  • Patient Outreach: Assist with patient outreach under clinical supervision, including regular outreach calls to provide patient and family support and education, and disease management instruction and education to patients and their families.
  • Caseload Management: Effectively manage caseloads through generating and monitoring reports.
Requirements:
  • Licensed Practical Nurse (LPN) license issued by the state in which the team member practices.
  • A valid driver's license issued by the Division of Motor Vehicles.
  • Completion of an accredited or approved program in Practical Nursing.
  • Typically requires 3 years of experience in a LPN role.
Working Conditions:

This position requires the ability to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday. Must be able to lift up to 50 lbs. from floor to waist, lift up to 20 lbs. over the head, and carry up to 40 lbs. a reasonable distance. Must be able to push/pull with 30 lbs. of force, perform a sliding transfer of 150 lbs. with a second person present, and operate all equipment necessary to perform the job.

May be exposed to mechanical, electrical, chemical, and radiation hazards as well as blood and body fluids; therefore, personal protective equipment must be worn as necessary.
