About the Position

The Housing Case Manager provides critical support to individuals experiencing homelessness, helping them achieve stable and permanent housing. This role involves assessing participant needs, developing personalized plans, and connecting them with essential resources and services.

Key Responsibilities
  • Provide trauma-informed case management, utilizing motivational interviewing and strength-based techniques to support participants.
  • Assess participant histories and resource/support needs, identifying opportunities for growth and stability.
  • Develop and implement plans to address barriers to housing stability, including mental health, substance use, and other challenges.
  • Collaborate with community providers and services to address participant needs, ensuring a comprehensive approach to housing stability.
  • Facilitate life skills training and workshops to support participants in achieving their goals.
  • Develop and maintain positive relationships with landlords, property managers, and other stakeholders to facilitate housing placement and retention.
Requirements
  • A minimum of 2 years' experience in a social/human service setting related to housing and homelessness.
  • Experience working with diverse populations, including individuals with mental health, substance use, and other challenges.
  • Strong communication and interpersonal skills, with the ability to work effectively with participants, community providers, and other stakeholders.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Knowledge of local resources and services for individuals experiencing homelessness or housing instability.
