Position Overview

HANAC, Inc. is a prominent social services organization established to address the needs of diverse populations across New York City. Our mission, rooted in community support, focuses on providing essential services to vulnerable groups.

The Case Worker for Extended Services plays a crucial role in assisting the Program Director with the execution of program guidelines aimed at enhancing client welfare. This position involves a variety of responsibilities designed to support our senior clients effectively.

Compensation: $18.00 per hour

Work Schedule: Monday to Friday, 9 AM to 2:30 PM (28 hours per week)

Key Responsibilities:

  • Conducting outreach calls to homebound seniors to maintain regular contact and perform needs assessments.
  • Visiting clients enrolled in the program to evaluate their circumstances and address any issues they may face.
  • Demonstrating a high level of empathy and understanding regarding the challenges faced by elderly individuals.
  • Utilizing strong listening skills and effective boundary-setting techniques.
  • Participating in monthly supervision meetings with the program supervisor or director to review client caseloads and report any changes in clients' physical, emotional, or environmental conditions.
  • Assisting seniors with understanding forms, notices, entitlements, and benefits, as well as completing applications and drafting correspondence as necessary.
  • Maintaining detailed service logs that document interactions with clients and relevant service information, ensuring accurate data entry into our systems.
  • Conducting follow-up activities to ensure the effective delivery of services and benefits.
  • Connecting elderly clients with local service providers and city agencies to facilitate access to necessary benefits and services.
  • Accompanying clients to city agencies when required, providing language support and ensuring they receive appropriate care and assistance.
  • Adhering to emergency protocols and collaborating with the Program Director in response to incidents or accidents.
  • Engaging in ongoing training and workshops to stay informed about policy changes and developments affecting senior services.

Qualifications:

  • High school diploma is mandatory.
  • Experience in the social services sector is preferred.
  • Bilingual proficiency in Spanish or Greek is advantageous.
  • Proficiency in data entry and familiarity with MS Office applications (Word, Excel) is required.
