Complex Primary Care Nurse Practitioner

We are seeking a highly motivated and experienced Nurse Practitioner to join our Complex Primary Care team at Dartmouth Health. The ideal candidate will have a strong interest in caring for patients with complex medical conditions and developmental disabilities.
The successful candidate will work in an outpatient setting, providing high-quality care and education to patients and their families. This is a full-time position with no call and limited site travel.
Key Responsibilities:
- Assess, diagnose, and manage complex and acute medical and behavioral health conditions
- Manage chronic medical conditions and provide ongoing care to patients
- Act as a patient advocate, ensuring that all aspects of patients' and families' needs are met
- Collaborate with primary care teams and specialists to provide comprehensive care
