POSITION OVERVIEW:

Under the supervision of the Behavioral Health Access & Outreach Division Manager, the Patient Navigators play a crucial role in assisting community members in accessing essential behavioral health and primary care services.


IDEAL CANDIDATE PROFILE:

The successful candidate will demonstrate strong cultural competency, possess program management experience, and have a background in assisting patients with behavioral health and healthcare services. They will be skilled in empathetic listening, problem-solving, and advocacy, ensuring patient concerns are effectively addressed.

The candidate will exhibit sound judgment and diplomacy, with a thorough understanding of organizational dynamics to engage the appropriate stakeholders.

Strong teamwork and relationship-building skills are essential for effectively supporting patients.


KEY RESPONSIBILITIES:

This role is distinguished by its focus on managing specialized programs, with responsibilities including:

  • Providing education to patients and families on navigating healthcare systems.
  • Advocating for patients experiencing challenges in accessing services.
  • Conducting community outreach to enhance understanding of behavioral and physical health conditions.
  • Building and maintaining partnerships with community organizations to support client treatment goals.
  • Managing budgets to connect patients with necessary community resources.

EDUCATIONAL AND EXPERIENCE REQUIREMENTS:

A bachelor’s degree in psychology, sociology, public health, or a related field is required, along with relevant work experience in mental healthcare or patient advocacy.

Additional qualifications may include experience in case management and knowledge of community resources for at-risk populations.
