Role Summary

The Triage Nurse plays a crucial role in supporting patients and their families by delivering clear, safe, and effective triage services, both via telephone and in-person. This position combines critical thinking with evidence-based tools to ensure standardized and safe care for our patient population.

Key Responsibilities

  1. Respond to incoming calls and visits promptly, assisting patients with standardized care and benefits navigation while establishing a professional rapport.
  2. Conduct comprehensive clinical assessments of symptoms to determine the appropriate level of care, referring patients according to established protocols.
  3. Adhere to standard operating procedures related to triage services.
  4. Communicate recommendations to patients in a clear and patient-friendly manner.
  5. Guide members to additional internal and external resources and community services as necessary.
  6. Act as the primary contact for emergency escalations for all members.
  7. Engage in team meetings and contribute to continuous quality improvement initiatives.
  8. Prepare patients for medical evaluations, including measuring and documenting vital signs and other relevant information in the Electronic Health Record.
  9. Assist medical providers during examinations and procedures.
  10. Collect blood samples and prepare specimens for laboratory testing.
  11. Perform quality control checks on laboratory equipment.
  12. Maintain and prepare medical equipment and rooms for patient visits.
  13. Monitor and request necessary medical supplies, ensuring compliance with OSHA standards.
  14. Maintain accurate records of external testing and ensure timely return of results.
  15. Implement medical provider orders, including notifying patients of test outcomes.
  16. Educate patients and their families regarding conditions, medications, and self-care techniques.
  17. Support colleagues with administrative tasks such as answering phones and patient registration as needed.
  18. Address patient complaints effectively.
  19. Perform other duties as assigned.

Qualifications and Skills

Required

  • Maintain patient confidentiality at all times.
  • Exhibit courtesy and respect towards patients and colleagues.
  • Demonstrate the ability to collaborate effectively with healthcare providers, staff, and third-party payors.
  • Possess strong organizational skills to prioritize and coordinate work activities independently.
  • Ability to perform under pressure.

Credentials and Experience

Required

  • High School Diploma or GED.
  • LPN certification.
  • A minimum of 2 years' experience in a triage environment, preferably with a focus on telephone and in-person triage, or 2 years in an emergency room, or 4 years in an ambulatory primary care role that included triage responsibilities.
  • Expertise in advanced clinical decision-making.
  • Comfortable managing a diverse range of medical conditions across pediatric and adult populations.

Preferred

  • Associate's or Bachelor's Degree in Nursing and an active, unrestricted Kentucky License as a Registered Nurse.

Work Environment

This role requires the ability to sit, stand, and walk for extended periods. It may involve working under stressful conditions or irregular hours, with frequent exposure to communicable diseases and other clinical environment factors. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
