Role Summary
The Triage Nurse plays a vital role in supporting patients and their families by delivering clear, safe, and effective triage services, both via telephone and in-person. This position requires the integration of critical thinking with evidence-based decision-making tools to ensure standardized and safe care for our diverse patient population.
Key Responsibilities
- Respond promptly to incoming inquiries and assist patients by providing standardized care and navigating benefits, while establishing a friendly yet professional rapport.
- Conduct comprehensive clinical assessments of symptoms and confidently determine the appropriate level of care needed to address the patient's medical requirements, referring them according to established protocols.
- Adhere to standard operating procedures and guidelines related to the triage service.
- Effectively educate and communicate recommendations to patients in a manner that is easy to understand.
- Successfully connect members to additional internal and external benefits and community resources as necessary.
- Act as a central point of contact for all emergency escalations related to Included Health members.
- Engage in team meetings and contribute to continuous quality improvement initiatives.
- Prepare patients for medical examinations, including measuring and recording vital signs, chief complaints, and other relevant information in the Electronic Health Record.
- Assist medical providers during examinations and procedures.
- Collect blood samples and prepare specimens for laboratory analysis.
- Perform quality control checks on laboratory equipment.
- Maintain and prepare medical equipment and examination rooms for patient visits.
- Monitor laboratory and patient care supplies, making requests for replenishment to the Supervisor or designated representative when necessary.
- Ensure compliance with OSHA standards in laboratory and patient care areas, including maintaining cleaning logs.
- Keep accurate records of external testing and ensure timely return of results.
- Implement medical provider orders, including notifying patients of test outcomes.
- Educate patients and their families regarding their conditions, medications, and self-care practices.
- Assist colleagues with answering phones, registering patients, and managing billing as needed.
- Address patient complaints appropriately.
- Perform other duties as assigned.
Required Knowledge, Skills, and Abilities
Essential
- Maintain patient confidentiality at all times.

- Demonstrate the ability to collaborate effectively with providers, staff, and third-party payors.
- Work independently, prioritize tasks, and coordinate work activities efficiently.

Qualifications and Experience
Essential
- High School Diploma or GED.
- Licensed Practical Nurse (LPN).
- A minimum of 2 years of experience in a triage environment, with a focus on telephone or in-person triage, or 2 years in an emergency room setting, or 4 years in an ambulatory primary care role that included triage responsibilities.
- Proficiency in advanced clinical decision-making.
- Comfortable managing a wide range of medical conditions across pediatric and adult populations.
Preferred
- Associate's or Bachelor's Degree in Nursing and an active, unrestricted Kentucky License as a Registered Nurse.
Special Considerations
This role requires the ability to sit, stand, and walk for extended periods. It may involve working under stressful conditions or irregular hours, and frequent exposure to communicable diseases, body fluids, and other typical clinic environment conditions. The employee must be able to reach with hands and arms and possess specific vision abilities, including close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. A moderate noise level in the work environment is expected.
