Job Description

We are seeking a highly skilled Clinical Pharmacist to join our team. The successful candidate will have a strong background in clinical pharmacy practice and excellent communication skills.

  • Provide pharmaceutical care to patients, including assessing their response to drug therapy and planning drug therapy based on physician-established diagnoses.
  • Collaborate with the medical staff to initiate, modify, or discontinue medications for ongoing therapy of chronic disease states.
  • Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics.
Requirements
  • Degree: Doctor of Pharmacy (Pharm.D.).
  • Education: Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
  • Internship/Residency: Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health-System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training, and CME in clinical pharmacy practice.
  • Experience: As required to meet clinical competency requirements specified in the Service-specific credentialing instructions. Possess one (1) year of full-time experience in the last three (3) years as a clinical pharmacist in an ambulatory care setting OR Completed an ASHP accredited Ambulatory Care Residency OR Possess Board Certification as a Pharmacotherapy Specialist.
  • Licensure: Current, full, active, and unrestricted license to practice as a pharmacist.
