The Residential Habilitation Coordinator/QIDP will oversee the provision of services for assigned individuals as determined by the Interdisciplinary Team. This role functions as an advocate for the individuals we support and reports directly to the RHC/QIDP Supervisor.

Key Responsibilities:
  • Participate as a member of the Interdisciplinary Team (IDT) to ensure comprehensive care planning.
  • Coordinate the Plan of Care for assigned individuals, schedule and facilitate IDT meetings, prepare reports, and submit them in a timely manner.
  • Notify families, correspondents, and staff regarding IDT meeting schedules and prepare Individual Staff Action Plans during semi-annual meetings.
  • Respond immediately to clinical issues that require attention.
  • Assist in training staff on implementing goals and services for assigned individuals.
  • Maintain contact with appointed guardians and family members to ensure effective communication and collaboration.
  • Evaluate and update service plans (DDP2, IPOP) as needed and ensure annual updates are completed.
  • Review and coordinate Life Plan updates with care management to ensure accuracy within the plan.
  • Conduct site observations to ensure the implementation of service plans and adherence to guidelines.

This role requires a Bachelor's Degree in a human service-related field and a minimum of one year of experience working with people with ID/DD, holding QIDP status, and possessing a valid NYS driver license.
