Position Overview:

We are seeking eight full-time Client Support Specialists to join our Adult Protective Services team. These roles will primarily support the Illinois Department on Aging in designated suburban areas.

Key Responsibilities:
  • Conduct thorough client assessments using intermediate-level skills, particularly for complex cases.
  • Develop tailored service plans in collaboration with clients, outlining specific goals and necessary services.
  • Provide advocacy, case management, and supportive counseling to individuals and families, assisting them in navigating environmental and interpersonal challenges.
  • Utilize an extensive knowledge of community resources to assist clients effectively.
  • Collaborate with community organizations and adhere to contractual and programmatic requirements, including timely reporting.
  • Regularly reassess client situations and the effectiveness of service plans.
  • Engage with program staff in the planning and execution of service strategies.
  • With supervisor approval, authorize limited financial assistance for clients.
  • Identify systemic issues and communicate them to the Program Director as necessary.
  • Participate in community outreach initiatives to enhance awareness of our services.
Qualifications:

A Bachelor's Degree in Social Work or a related field from an accredited institution is required. Candidates should have at least one year of case management experience in a social service or healthcare environment.

Successful candidates will demonstrate the ability to provide effective services to clients, manage their responsibilities efficiently, and work well with diverse populations. Flexibility to work evenings and weekends is essential.

Physical Requirements:

This role requires the ability to sit and stand for extended periods, communicate effectively, and occasionally lift up to 25 pounds. Close visual acuity is necessary for tasks such as data analysis and computer work.

Compensation:
