Job Overview:
The Clinical Pharmacist is responsible for delivering high-quality pharmaceutical care, ensuring the provision of safe, effective, and economical medication therapy for all patients. This role involves developing, recommending, and monitoring evidence-based pharmacotherapy plans with a focus on enhancing patient outcomes and quality of life. The pharmacist will assess patients' medications for appropriateness, effectiveness, safety, and adherence, while also monitoring relevant patient and laboratory parameters.

Key Responsibilities:
  • Oversees the delivery of pharmaceutical care within the designated patient care area.
  • Coordinates resources to ensure timely, efficient, and appropriate care.
  • Interprets and clarifies medication orders from physicians.
  • Reviews patient medication profiles to identify potential drug sensitivities and interactions.
  • Evaluates all medication orders for prescribing and therapeutic appropriateness.
  • Recommends suitable formulary alternatives based on clinical assessments.
  • Consults with medical staff regarding drug therapy issues and monitors patient laboratory results.
  • Provides comprehensive drug information to healthcare professionals.
  • Conducts patient discharge medication consultations in line with departmental standards.
  • Participates in rounds with medical teams.
  • Calculates drug dosages using pharmacokinetic principles.
  • Verifies provider-entered orders to ensure accurate dispensing.
  • Delivers educational sessions to healthcare providers as needed.
  • Prepares specialized medications through compounding and sterile techniques.
  • Communicates supply needs to maintain adequate stock levels.
  • Supervises pharmacy technicians, students, and interns, ensuring the accuracy of their work.
  • Collaborates with medical and nursing staff to resolve pharmacy service issues.
  • Coordinates with pharmacy personnel to achieve departmental goals.
  • Assists in reviewing and updating departmental policies and procedures.
Work is primarily conducted in a clinical setting. The Clinical Pharmacist is accountable for fulfilling job-specific responsibilities and adhering to organizational policies and procedures. Position Highlights:

Educational Requirements:
Bachelor's Degree in Pharmacy (Required), Doctoral Degree (Preferred)
