Job Summary:
The Clinical Pharmacist is responsible for delivering high-quality pharmaceutical care, ensuring the provision of safe, effective, and economical medication therapy for all patients.
This role involves developing, recommending, and monitoring evidence-based pharmacotherapy plans with a focus on enhancing patient outcomes and overall quality of life.
These plans encompass the evaluation of patient medications for appropriateness, effectiveness, safety, and adherence, along with monitoring patient and laboratory parameters, documentation needs, and outcomes.
Key Responsibilities:
Assumes accountability for pharmaceutical care delivery in the designated patient care area.
Coordinates the utilization of human, material, and informational resources to ensure timely, efficient, and appropriate care.
Interprets and clarifies physicians' medication orders.Reviews patient medication profiles to identify potential drug sensitivities, contraindications, duplications, and significant drug interactions.
Evaluates all medication orders for prescribing, formulary, and therapeutic considerations.
Recommends suitable formulary alternatives based on clinical parameters.
Consults with medical staff regarding specific drug therapy issues and monitors patient laboratory results.
Provides comprehensive drug information and educational materials to healthcare professionals.Conducts patient discharge medication consultations in accordance with departmental standards.
Participates in rounds with medical teams.
Calculates drug dosages based on pharmacokinetic principles.
Verifies provider-entered orders and ensures accurate dispensing.
Delivers in-service training to healthcare providers as needed.
Prepares specialized medications through compounding and sterile techniques.
Communicates supply and equipment needs to maintain stock levels.
Oversees workplace activities and resources; promotes professional development; ensures compliance with departmental standards.
Mentors pharmacy technicians, students, and interns; assumes final responsibility for their work accuracy.
Collaborates with medical and nursing staff to address pharmacy service-related issues.Coordinates with pharmacy personnel to achieve departmental and hospital goals.
Assists in reviewing and updating departmental policies and procedures as necessary.

Education:
Bachelor's Degree in Pharmacy (Required), Doctoral Degree (Preferred)
