Job Summary

North Suffolk Mental Health Association is seeking a highly skilled Care Navigator to join our team. The successful candidate will be responsible for coordinating case navigation, participation, and transition for all CBHC programming.

Key Responsibilities
  • Case Navigation: Coordinate and perform referrals in a prompt, comprehensive, and responsive manner, including communication with referral sources, collateral providers, and community partners.
  • Client Support: Participate in outreach and engagement activities with program participants, including follow-up appointments, and provide education and support throughout the process.
  • Administrative Tasks: Run client insurance in real-time, reconcile cases from call logs with state programs, and complete bed searches for CCS, CBAT, detox, and other acute levels of care as needed.
  • Risk and Crisis Management: Identify risk and behavioral crisis needs and bring them to the attention of clinical staff as needed.
  • Documentation and Reporting: Complete clinical documentation as required in the electronic health record and/or other databases, and maintain databases and spreadsheets for information tracking, communication, and client assignments.
  • Collaboration and Communication: Collaborate with clinical staff to develop and implement individual action plans, monitor and report progress, and document as appropriate to credential level.
Requirements
  • Education and Experience: Bachelor's Degree in Social Work, Counseling Psychology, Psychology, Expressive Therapies, or related field of study, and a minimum of 2 years post-degree experience within a multidisciplinary treatment setting.
  • Skills and Qualifications: Group facilitation skills and experience working with individuals with substance use disorders required.
