About the Role

Kaiser Permanente is seeking a skilled Social Worker to join our team in Clackamas, Oregon.

Key Responsibilities
  • Provide social work services to support members in directing their own healthcare experience.
  • Assess and intervene to optimize member health outcomes.
  • Link members to health plan and community resources.
  • Collaborate with medical teams to understand patient beliefs, behaviors, and barriers.
  • Develop cost-effective health care plans with members.
  • Promote patient-centered, multi-disciplinary care.
  • Maintain thorough knowledge of Kaiser Permanente services, community resources, and relevant laws.
  • Complete required documentation and participate in program development.
Requirements
  • Minimum one year of Social Work experience in a hospital, medical outpatient clinic, home health/hospice, or long-term care setting.
  • Successful completion of the Kaiser Permanente NW Social Worker Internship program.
  • Masters degree in social work from an accredited university.
  • Masters Social Worker License (Oregon) within 6 months of hire OR Clinical Social Work Associate Certificate (Oregon) within 6 months of hire OR Licensed Clinical Social Worker (Oregon) within 6 months of hire.
Preferred Qualifications
  • Minimum three years of professional Social Work experience in a hospital, medical outpatient clinic, home health/hospice, or long-term care setting.
  • Expertise with short-term counseling interventions.
  • Advanced knowledge of local and national resources related to healthcare.
