We are seeking a highly skilled and compassionate Nurse Practitioner - Advanced Care to join our team at Optum. As a key member of our Senior Community Care (SCC) program, you will play a vital role in delivering high-quality, patient-centered care to our most vulnerable populations.
Responsibilities
- Conduct comprehensive assessments of patients' medical and social needs
- Develop and implement individualized care plans to address patients' complex health needs
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Provide education and support to patients and their families to promote healthy behaviors and self-management
- Participate in quality improvement initiatives to enhance patient outcomes and experiences
Requirements
- Master's degree in Nursing or Doctor of Nursing Practice
- Active and unrestricted license to practice as a Nurse Practitioner in the state of employment
- Board certification as a Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner
- Current DEA licensure/prescriptive authority or ability to obtain post-hire
- Ability to gain a collaborative practice agreement, if applicable in your state
- Access to reliable transportation
Preferred Qualifications
- 1+ years of clinical experience in a long-term care setting
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end-of-life discussions
- Proficient computer skills, including the ability to document medical information with electronic medical records
