Key Responsibilities:
- Coordinate post-discharge patient care needs to ensure a seamless transition to lower levels of care.
- Conduct thorough patient assessments independently to identify potential care gaps and develop effective care plans.
- Collaborate with patients, families, and healthcare teams to develop and implement care plans that meet individual needs.
- Resolve issues and concerns to ensure safe transfers to lower levels of care, promoting continuity of care and minimizing readmissions.
- Maintain ongoing communication with interdisciplinary staff to ensure timely and effective care coordination.
- Ensure regulatory compliance and adherence to industry standards and best practices.
- Perform other duties as assigned to support the delivery of high-quality patient care.
Requirements:
- Minimum of two years RN experience in various healthcare settings, with a strong understanding of care processes and resources.
- Successful completion of an RN program and current Registered Nurse License.
- Basic Life Support certification required.
- Ability to work in a team environment, advocate for patients, and navigate conflicts effectively.
- Effective communication and problem-solving skills, with the ability to adapt to changing situations.
- Cultural competency and knowledge of care processes and resources.
- Proficiency with technology and ability to navigate electronic health records.
Preferred Qualifications:
- Knowledge of healthcare resources utilization and understanding of Medicare and Medicaid regulations.
- Case Management certification preferred.
- BSN or higher degree, with a strong foundation in nursing and healthcare management.
