  • Resolve issues and concerns to ensure safe transfers to lower levels of care, promoting continuity of care and minimizing readmissions.
  • Maintain ongoing communication with interdisciplinary staff to ensure timely and effective care coordination.
  • Ensure regulatory compliance and adherence to industry standards and best practices.
  • Perform other duties as assigned to support the delivery of high-quality patient care.

Requirements:

  • Minimum of two years RN experience in various healthcare settings, with a strong understanding of care processes and resources.
  • Successful completion of an RN program and current Registered Nurse License.
  • Basic Life Support certification required.
  • Ability to work in a team environment, advocate for patients, and navigate conflicts effectively.
  • Effective communication and problem-solving skills, with the ability to adapt to changing situations.
  • Cultural competency and knowledge of care processes and resources.
  • Proficiency with technology and ability to navigate electronic health records.

Preferred Qualifications:

  • Knowledge of healthcare resources utilization and understanding of Medicare and Medicaid regulations.
  • Case Management certification preferred.
  • BSN or higher degree, with a strong foundation in nursing and healthcare management.
