Join Our Team as an Advanced Practice Nurse

Providence Hospice and Palliative Care is in search of a dedicated Advanced Practice Nurse to join our compassionate team. As a division of Providence Life Services, we are committed to delivering a comprehensive range of healthcare services, particularly focused on senior living and end-of-life care.

About the Role:
The Advanced Practice Nurse will operate both independently and collaboratively with physicians to deliver primary care to patients. This role involves overseeing, conducting, and completing all necessary visits and documentation in alignment with CMS hospice regulations for face-to-face encounters.

Key Responsibilities:
1. Manage the care of patients in palliative settings by conducting initial and follow-up visits alongside social workers and nursing staff.
2. Perform patient assessments, gather, and analyze diagnostic information.
3. Conduct comprehensive evaluations of patients' health conditions to provide precise clinical data for hospice eligibility.
4. Collaborate in enhancing medical care management, including prescribing medications, ordering laboratory tests, and coordinating with various healthcare professionals.

Communication:
Encourage transparent communication among interdisciplinary team members, patients, and families to ensure that end-of-life care goals are met effectively.

Coordination of Care:
Assess and prioritize patient needs, placing orders and liaising with medical staff, nursing personnel, social services, and other relevant parties to coordinate a comprehensive care plan.

Qualifications:
- Must hold a valid Nurse Practitioner certification in the State of Indiana.
- A minimum of 1-2 years of experience, preferably in Home Health or Hospice settings.
