Mental Health Care Specialist

At Mental Health Partners, we are dedicated to delivering high-quality mental health services to our community. We are currently seeking a highly skilled and compassionate Care Coordinator to join our team.

As a Care Coordinator, you will play a vital role in providing support and guidance to individuals struggling with severe mental illness and homelessness. Your primary responsibility will be to work closely with clients, their families, and healthcare providers to develop and implement personalized care plans that promote recovery and stability.

About the Role:

  • Provide comprehensive case management services to clients, including assessment, planning, and coordination of care.
  • Develop and maintain relationships with community resources, service providers, and other stakeholders to ensure seamless care transitions.
  • Collaborate with multidisciplinary teams to coordinate care, share knowledge, and enhance client outcomes.
  • Participate in quality improvement initiatives to identify areas for growth and implement evidence-based best practices.
  • Engage in ongoing professional development to stay current on industry trends, research, and best practices.

Requirements:

  • Bachelor's degree in Psychology, Social Work, Counseling, or a related field.
  • One year of experience working in the behavioral health field, preferably in an outpatient setting.
  • Strong communication, interpersonal, and problem-solving skills.
  • Ability to work effectively in a fast-paced environment, prioritize tasks, and manage multiple responsibilities.
  • Current Colorado driver's license and safe driving record.
