Mental Health Support Coordinator

We are seeking a highly skilled Care Coordinator to join our team. As a key member of our multidisciplinary team, you will be responsible for coordinating patient care, developing treatment plans, and ensuring seamless delivery of services.
Key Responsibilities- Coordinate patient care, including scheduling appointments and follow-up visits.
- Develop and implement individualized treatment plans, taking into account the patient's needs, goals, and preferences.
- Collaborate with healthcare professionals, family members, and caregivers to ensure a comprehensive approach to patient care.
- Conduct thorough assessments, identify areas for improvement, and develop strategies to address them.
- Bachelor's degree in Psychology, Social Work, Sociology, or a related field.
- One year of experience in the behavioral health field, preferably in an outpatient setting.
- Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
- Ability to work collaboratively as part of a multidisciplinary team, promoting a culture of respect, empathy, and understanding.
The estimated hourly salary for this role is $20.35 - $25.44, based on experience.
