About the Role

We are seeking a highly skilled Care Coordinator to join our team. As a key member of our multidisciplinary team, you will be responsible for coordinating patient care, developing treatment plans, and ensuring seamless delivery of services.

Key Responsibilities
  1. Coordinate patient care, including scheduling appointments and follow-up visits.
  2. Develop and implement individualized treatment plans, taking into account the patient's needs, goals, and preferences.
  3. Collaborate with healthcare professionals, family members, and caregivers to ensure a comprehensive approach to patient care.
  4. Conduct thorough assessments, identify areas for improvement, and develop strategies to address them.
What You'll Need
  • Bachelor's degree in Psychology, Social Work, Sociology, or a related field.
  • One year of experience in the behavioral health field, preferably in an outpatient setting.
  • Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
  • Ability to work collaboratively as part of a multidisciplinary team, promoting a culture of respect, empathy, and understanding.
Compensation and Benefits

The estimated hourly salary for this role is $20.35 - $25.44, based on experience.
