Job Description:

We are seeking a motivated and detail-oriented Behavioral Health Case Manager to join our team. As a key member of our outreach program, you will work closely with clients, families, and community agencies to provide case management services and promote healthy behaviors.


Requirements:
  • Bachelor's degree in Psychology, Social Work, Sociology, or a related field
  • Experience working with adults in an outpatient setting
  • Strong community outreach experience and desires
