Mental Health Partners is seeking a skilled Behavioral Health Professional to join our team. In this role, you will provide quality assessments of health home clients care needs and facilitate timely access to care.

Salary and Benefits

$26.51 - $30.58 per hour, plus comprehensive benefits package including medical, dental, and vision insurance, retirement savings plan, and paid time off.

Job Description

Critical Job Functions:

  • Conduct comprehensive assessments of new and returning clients to identify their behavioral health needs.
  • Create and oversee care plans to ensure timely access to care and superior customer service.
  • Maintain clinical oversight of clients assigned to the Behavioral Health Home (BHH) team.
  • Sign off on documentation from other team members as needed to ensure quality of care and medical necessity.
  • Identify high-risk or urgent clients and provide triage to get them in quickly for services.
  • Assess any needs for special reports, letters, or applications, and delegate tasks to Client Service Advisors when necessary.
  • Review care plans and treatment progress with Care Coordinators and identify next steps or follow-up needed.

Requirements:

  • Masters in Psychology, Social Work, Counseling, or related field, plus 1-2 year internship, and 3-7 years prior experience in a clinical setting.
  • Licensed PhD or PsyD, Licensed Clinical Social Worker (LCSW), or Licensed Professional Counselor (LPC) required.
  • Experience providing clinical assessments and brief intervention.
