Foster patient/family self-management and compliance with individualized plan of care via post-acute phone call.

Collaborate with patient/family, physicians, and interdisciplinary team to facilitate patient's progress through acute episode of care in efficient and cost-effective manner.

Initiate evidence-based protocols and order sets according to patient's admission diagnosis, influencing progression of care and proposing appropriate transition plan within 48 hours of admission.

Responsibilities

  • Advocate for patient and family throughout entire care continuum
  • Facilitate communication with multidisciplinary team, referring physician, and ambulatory care team
  • Develop and implement individualized plan of care for patients
  • Ensure seamless transition of care from inpatient to post-acute setting

Requirements

  • Bachelor's Degree: BSN (Required)
  • 3 Years experience in nursing in acute care setting (Required)
  • Previous experience in clinical healthcare setting or insurance setting responsible for direct management of patient care (Preferred)
  • RN-LIC - PA Registered Nurse License (Required)
  • CCM - Certified Case Manager (Preferred)
