Job Title: Nurse Navigator - Comprehensive Vascular Anomalies Program

Children's Hospital of Philadelphia (CHOP) is seeking a highly skilled Nurse Navigator to join our Comprehensive Vascular Anomalies Program. As a key member of our team, you will provide guidance, oversight, and coordination to ensure seamless care for patients and families.

Responsibilities:
  • Represent patients and families from local, regional, and national sources, coordinating their care and providing ongoing support.
  • Coordinate patient care services, maintaining interpersonal relationships with patients, families, physicians, and referral sources.
  • Work with leadership to develop and implement education and outreach activities for the program.
  • Collaborate with stakeholders to strengthen relationships and facilitate the referral process.
  • Establish and maintain metrics, tracking referrals and providing data-driven insights to support departmental growth objectives.
Requirements:
  • Technical Diploma in Nursing required; Master's Degree preferred.
  • At least five years of nursing experience, with previous experience in pediatrics required.
  • Excellent verbal and written communication skills, with the ability to convey complex information in a clear and concise manner.
  • Ability to work independently with minimal supervision, with a strong focus on customer service and interpersonal skills.
