Job Summary

The Nurse Care Coordinator at Jefferson Health plays a vital role in ensuring seamless patient care transitions. This position requires a skilled nurse to coordinate and facilitate the continuum of care for patients, from hospital admission to discharge and beyond.

Key Responsibilities

  • Develop and implement comprehensive care plans that address the unique needs of each patient.
  • Collaborate with healthcare teams to ensure timely and effective communication, promoting patient-centered care.
  • Identify and mitigate barriers to care, ensuring patients receive the necessary resources and support for a successful transition.
  • Monitor patient progress, intervening as necessary to ensure efficient and effective care.
  • Escalate unresolved issues to department leadership, ensuring that patient needs are met.

Requirements

  • Bachelor's degree in Nursing required.
  • Minimum of 3 years of experience in case management or discharge planning.
  • Active relevant Registered Nurse license in Pennsylvania/New Jersey.
