Pediatric Nurse Navigator Job Description

About the Role

The Children's Hospital of Philadelphia (CHOP) is seeking a highly skilled Pediatric Nurse Navigator to join our Comprehensive Vascular Anomalies Program. As a key member of our team, you will provide guidance, oversight, and coordination to ensure seamless patient care and exceptional outcomes.

Key Responsibilities

* Represent patients and families from local, regional, and national sources, coordinating their care and providing ongoing support and education.
* Develop and implement strategies to increase utilization of our services, programs, and physicians, identifying new markets and evaluating their potential.
* Collaborate with stakeholders at all levels, including referring physicians, hospitals, and community agencies, to strengthen relationships and facilitate the referral process.
* Establish and maintain metrics to track program performance, providing regular reports and analyses to support departmental growth objectives.
* Act as a resource and mentor for staff, providing guidance on patient and family processes, and complex patient issues.

Requirements

* At least five (5) years of nursing experience, with a focus on pediatrics and vascular anomalies.
* Expert knowledge of pediatric nursing, with intermediate proficiency in office software, including Microsoft Office, and basic knowledge of database programs and data management concepts.
* Excellent verbal and written communication skills, with the ability to convey complex information in a clear and concise manner.
* Ability to work independently with minimal supervision, with a strong focus on customer service and interpersonal skills.
