About the Role

We are seeking a highly skilled Nurse Practitioner to join our team at Curo Health Services. As a key member of our Advanced Illness Management team, you will play a critical role in delivering high-quality, patient-centered care to individuals with complex medical needs.

Key Responsibilities
  • Collaborate with our interdisciplinary team to develop and implement comprehensive care plans that address the physical, emotional, and social needs of our patients.
  • Provide advanced illness management and palliative care services, including diagnosis, treatment, and follow-up care, under the direction of our Palliative Medical Director or Primary Care physician.
  • Work closely with patients, families, and caregivers to ensure that their needs are met and that they receive the highest level of care possible.
  • Participate in evening and weekend call rotations, providing on-call services in a clinically competent and responsive manner.
  • Prescribe medications, including controlled substances, as delegated and licensed, and order and interpret laboratory, imaging, and other diagnostic tests within the scope of professional practice.
  • Communicate clinical findings and treatment plans to our team, as needed, and re-assess the effectiveness of care plans on a regular basis.
Requirements
  • Master's degree in Nursing from an accredited school of nursing or equivalent as allowed under state law.
  • Minimum of two years of experience as a Nurse Practitioner, with a specialty in Family or Adult Medicine preferred.
  • Value-Based Care practice experience and Part B billing experience are also preferred.
  • Current license as a registered nurse or equivalent as required by state law for Nurse Practitioner licensure.
  • Current Nurse Practitioner certification/license and Collaborative agreement with a supervising physician.
  • Part B Provider certification, Federal DEA license, and State DEA license (if required).
