Job Title: Licensed Practical Nurse - Clinical Support Staff

Job Summary:

At 194660 Axess Family Services, we are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Clinical Support Staff. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and supporting our medical staff in providing exceptional service to our patients.

Key Responsibilities:

  • Pre-Visit Planning: Conduct thorough pre-visit planning for each scheduled patient appointment, ensuring that all necessary information and documentation are in place.
  • Assisting Providers: Assist medical providers with patient office visits, including rooming patients, documenting patient history, and recording vital signs.
  • Medication Administration: Prepare, administer, and document medications and/or procedures as directed by the medical provider.
  • Health Education: Provide counseling and health education to patients, offering necessary literature and instruction for continuity of care.
  • OSHA Compliance: Follow all OSHA regulations for hazardous materials, ensuring a safe working environment for patients and staff.
  • Exam Room Preparation: Prepare exam rooms, including cleaning after each patient, upkeep, and re-stocking of supplies.
  • Patient Safety: Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Team Collaboration: Work effectively within a team environment by exchanging honest and constructive feedback, supporting team goals, encouraging other team members, and collaborating with patient care team members to consolidate work, solve problems, create efficiencies, and reduce costs.
  • Accountability: Assume accountability for demonstrating behaviors consistent with API's written policies and procedures.
  • Physical Demands: Ability to lift, carry, and store clinical equipment and supplies within the center as directed.
  • Communicable Diseases: Follow Health Department guidelines regarding communicable diseases.
  • Electronic Health Record: Document all nursing care consistent with professional standards utilizing the agency-approved electronic health record.
  • Communication: Communicate appropriate information, both verbal and written, diplomatically with patients and staff, including those which may be of a sensitive nature to ensure continuity of care.
  • Phone and Voicemail: Address all incoming phone calls, patient requests, and voicemail throughout the day.
  • Confidentiality: Protect confidential information of patients from unauthorized use or disclosure.
  • Inventory Management: Coordinate inventory, order processing, and distribution of products and services.
  • Task Distribution: Ensure appropriate distribution of tasks within your team.
  • Department Meetings: Participate in daily huddles and attend regular department meetings, effectively training and educating the team.
  • Accuracy and Attention to Detail: Ensure daily duties, logs, and checklists are being completed accurately.
  • Compliance: Adhere to departmental policies and procedures.
  • Child Abuse Reporting: Per Ohio Revised Code, report any suspected child abuse or neglect to child protective services or law enforcement.

Requirements:

  • Education: Graduation from an accredited nursing program or certification as a medical assistant.
  • Licensure: Holds and maintains an unrestricted LPN license in the State of Ohio or holds a certified medical assistance certification.
  • Certification: Holds and maintains BLS certification through the American Heart Association.
  • Language Skills: Ability to read, write, and speak English.
  • Experience: Two to five years of community health experience preferred.
  • Computer Competency: Computer competency to include use of word processing, emails, and internet.
  • Electronic Health Record Experience: Previous experience with electronic health record technology is preferred.

Licenses or Certifications Required:

  • LPN License - State of Ohio or
  • Certification in Medical Assisting

Compliance:

This position requires compliance with API's written standards, including its Compliance Program and Standards of Conduct and policies and procedures. Such compliance will be an element considered as part of the regular performance evaluation.

Failure to comply with API's Written Standards, which may include the failure to report any conduct or event that potentially violates legal or compliance requirements or API's Written Standards or, for managers and supervisors, fails to detect non-compliant conduct where reasonable efforts would have resulted in detection, will be met by the enforcement of disciplinary action, up to and including possible termination, in accordance with API's Compliance Program Policy and Procedure: Addressing Instances of Non-Compliance through Appropriate Disciplinary Actions.

The noise level in the work environment is low to moderate. Stress level can be high at times.
