We are seeking a highly skilled Clinical Pharmacist to join our team at Sutter Health in Palo Alto Medical Foundation. This role is ideal for an individual with a passion for oncology and a strong background in medication therapy.

Job Summary

This position will work independently and in consultation with physicians and other healthcare providers across the healthcare continuum, ensuring appropriate medication therapy for individual patients.

  • Main Responsibilities:
  • Uses patient interviews and counseling, reviews medical records and medication profiles, consults with physicians.
  • Teaches, provides staff education, conducts medication use evaluations, monitors adverse reactions, and resolves medication-related problems.
  • Targets high-risk populations through medication reconciliation and comprehensive medication use management.
  • Assists with care coordination, disease state education, and monitoring.

Requirements:

  • Education: Doctorate from an accredited school of pharmacy.
  • Typical Experience: 5 years recent relevant experience.
  • Certification & Licensure: PHARMD-Licensed Pharmacist or PHARMD-Registered Pharmacist.
