Job Overview:Compensation: $29.00 per hour
Shift Differentials for Overnight and Weekend Positions
Available Shifts: One day position from 7 AM to 7:30 PM and one overnight position from 7 PM to 7:30 AM
Key Responsibilities:- Clarifies, interprets, and coordinates patients' rights and advanced directives; explains Medicare Hospice Benefits when necessary.
- Conducts ongoing physical and psychosocial evaluations of hospice patients as directed by the Registered Nurse.
- Implements nursing care plans for assigned patients and their families, utilizing appropriate treatment modalities.
- Exhibits strong organizational, time management, and prioritization skills.
- Records and observes signs and symptoms related to the disease process.
- Communicates effectively with physicians, families, and other healthcare team members to report changes in patients' conditions and identify their needs.
- Performs technical procedures in accordance with the LPN Nurse Practice Act and agency policies.
- Encourages and supports patient and family autonomy and independence.
- Prepares and administers medications as per policy and documents accordingly.
- Provides personal care and assists with activities of daily living as directed.
- Documents vital signs, nursing interventions, treatments, and patient responses; completes necessary documentation as per agency policy.
- Collaborates with the Resident Administrator and other healthcare team members in planning, implementing, evaluating, and coordinating services for patients and families to ensure continuity of care.
- Completes chart documentation and rounding before leaving the facility.
- Participates in Emergency Preparedness and Disaster Plans as required.
- Maintains proper infection control practices.
- Ensures confidentiality of all patient information, disclosing it only to authorized individuals.
- Engages in all mandatory in-service education programs and meetings.
- Adheres to the dress code policy by maintaining a professional appearance and wearing an ID badge while on duty.
- Performs additional duties as assigned.
- Strong nursing assessment and documentation capabilities.
- Clinical knowledge to support end-of-life care.
- Ability to act as a resource for patients and families during challenging times.
- Excellent interpersonal and communication skills.
- Proficient in both spoken and written English, ensuring clear communication.
- Well-organized to manage patient and family needs while overseeing hospice operations.
- Proficient in computer usage, including Microsoft applications (Word, Excel, Outlook) and electronic medical records.
Required Education: Graduation from an accredited Practical Nursing program; licensure as a Practical Nurse in New Jersey; licensure in Pennsylvania if practicing in PA. CPR/AED Certification.
Preferred Education:
Required Experience: Minimum one year of clinical experience in adult or pediatric medical/surgical settings.
Preferred Experience: Previous experience in home care or hospice settings is preferred.
