Position Overview

The Patient Care Liaison - Licensed Practical Nurse (PCC-LPN) plays a pivotal role in the coordination of patient care in collaboration with the healthcare team. This position involves the support of personalized care strategies, addressing patient risk factors, and enhancing communication between the Primary Care Physician and the patient.

  • Develop and implement tailored patient care strategies
  • Identify and address patient risk factors and barriers to care
  • Enhance communication between PCP and patient
  • Engage in care coordination meetings
  • Manage a patient caseload effectively
  • Utilize sound clinical judgment in decision-making
  • Contribute to quality improvement initiatives

Qualifications

- Valid NJ Practical Nurse License
- Minimum of two (2) years of experience in healthcare environments
- Possession of a valid driver's license and willingness to travel
