Position Overview
Starting Compensation: $45 per hour
Flexible schedule based on business needs: 2pm-10pm
Key Responsibilities:
- Facilitates and clarifies patients' rights and advanced directives; provides information on Medicare Hospice Benefits when applicable.
- Conducts admissions, follow-up visits, and pronouncements.
- Adheres to the established care plan for each patient and communicates observations to the team leader or primary nurse.
- Collaborates with the Interdisciplinary Team (IDT) regarding new nursing diagnoses and necessary adjustments to the care plan.
- Works with the team concerning hospice aides and LPN staff.
- Assists patients and families in acquiring essential durable medical equipment and supplies.
- Monitors and documents signs and symptoms of the disease process.
- Engages with physicians, families, and other healthcare team members to report changes in the patient's physical and emotional condition and identify patient needs.
- Provides education to patients and families; utilizes educational tools and handouts; documents all teaching and patient responses.
- Encourages and supports patient and family self-direction and independence.
- Prepares and administers medications in accordance with policy and documents accordingly.
- Offers support to the primary caregiver and delivers direct care to the patient.
- Completes chart documentation within 48 hours.
- Records vital signs, measurements, nursing interventions, treatments, and patient responses; completes flow sheets, narratives, and verbal orders as per agency policy.
- Maintains productivity in compliance with agency standards.
- Communicates with the Nursing Supervisor and other health team members in planning, implementing, evaluating, and coordinating services to ensure completeness and continuity of care.
- Participates in the on-call system as required.
- Incorporates the physiological and developmental age of patients into the overall patient care plan.
- Adheres to infection control practices.
- Maintains confidentiality of all patient information and discloses information only to authorized individuals.
- Takes clinical calls based on business needs.
- Provides clinical and clerical support as needed.
- Participates in all required in-service education programs and meetings, attending 9 out of 12 nursing meetings annually.
- Demonstrates compliance with the dress code policy by appearing well-groomed, wearing an ID badge while on duty, and maintaining a professional appearance.
- Performs additional duties as assigned.
Required Skills/Abilities:
- Strong nursing assessment and documentation skills.
- Clinical knowledge to support the end-of-life process.
- Ability to serve as a resource for patients and families during challenging times.
- Excellent interpersonal and communication skills.
- Ability to comprehend spoken and written English, communicating clearly both verbally and in writing.
- Well-organized to manage patient and family needs, providing clinical oversight of the inpatient hospice operation.
- Proficiency in computer operations, including knowledge of Microsoft applications (Word, Excel, and Outlook) and electronic medical records.
Required Education: Diploma, Associate or Bachelor of Science in Nursing; BSN preferred. Licensed to practice as a Registered Nurse in the State of New Jersey; licensure in Pennsylvania if practicing in PA.
Preferred Education: CPR certification preferred.
Required Experience: Minimum one year of clinical experience in adult, pediatric, or medical/surgical settings.
Preferred Experience: Prior experience in home care or hospice preferred.
