Position Overview:
UPMC Altoona is seeking a dedicated Oncology Registered Nurse to serve as an Oncology Patient Navigator within the Radiation Oncology department. This role operates during standard business hours, Monday through Friday.

Key Responsibilities:
The Oncology Patient Navigator plays a crucial role in providing comprehensive assessments, counseling, and coordination for oncology patients throughout their cancer diagnosis and treatment journey.

Patient Advocacy:
Possesses in-depth knowledge of reimbursement systems and effectively educates patients and families on payer requirements and coverage for post-hospital services. Advocates for patients to secure necessary authorizations.

Risk Management:
Identifies practices or events that may compromise patient safety and reports findings to the appropriate hospital authorities.

Documentation and Quality Assurance:
Ensures adherence to standards of care through concurrent case reviews and monitoring for quality issues. Makes referrals to relevant professionals when standards are not met.

Telemedicine Coordination:
Facilitates service coordination via telemedicine, including scheduling, patient assessments, and follow-ups as needed.

Patient Education:
Educates and advocates for patients and their families, empowering them to make informed decisions regarding medical care, discharge planning, advance directives, and end-of-life considerations.

Community Resource Navigation:
Maintains extensive knowledge of federal, state, and local assistance programs and community resources to address patient needs. Provides resource-specific information and facilitates referrals.

Comprehensive Assessments:
Conducts psychosocial assessments that encompass various factors, including social, economic, cultural, and behavioral aspects, along with histories of abuse and support systems.

Collaboration:
Works closely with healthcare teams, patients, families, and community resources to ensure that oncology patients are well-informed and actively engaged in their care plans.

Qualifications:
- Bachelor's degree in Nursing with 7-10 years of clinical experience.
- Required experience in Oncology.
- Preferred experience in Case Management.
- OCN certification is a plus.

Licensure and Certifications:
- Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification required.
- Current Registered Nurse (RN) licensure.
