The Oncology Patient Navigator plays a crucial role in conducting assessments, providing counseling, facilitating communication, and coordinating care for oncology patients throughout their cancer diagnosis and treatment journey.


Key Responsibilities:


Exhibits a comprehensive understanding of reimbursement frameworks, enabling the education of patients and families about payer requirements and coverage for post-hospital services, while effectively advocating for patients to secure necessary authorizations.

Maintains professional integrity by respecting confidentiality and honoring patient autonomy.


Risk Management:
Capable of identifying practices or incidents that may compromise patient safety.

Reports findings to the hospital's legal counsel, Risk Manager, or Quality Assurance Director as required.

Documentation and Quality Assurance:
Skilled in recognizing appropriate care standards.

Conducts ongoing case evaluations and monitors for quality concerns.

Refers cases to the Physician Advisor or Risk Manager when care standards are not met.

Utilizes telemedicine to coordinate services, including scheduling, patient evaluations, and follow-ups as necessary.

Identifies, assesses, counsels, and intervenes effectively with patients facing complex medical and psychosocial challenges.


Educates and advocates for patients and their families, empowering them to make informed choices regarding medical treatment, discharge planning, advance directives, and end-of-life care.


Possesses extensive knowledge of federal, state, and local assistance programs, as well as community resources that impact patient needs; demonstrates proficiency in navigating social systems by providing targeted resource information to patients and families, initiating contact with relevant resources, and facilitating referrals.


Risk Management:
Adept at identifying practices or events that may endanger patient safety.

Reports findings to the hospital's legal counsel, Risk Manager, or Quality Assurance Director as necessary.


Conducts comprehensive psychosocial assessments, considering social, economic, cultural, age-related, and behavioral factors, along with histories of abuse, prior psychiatric or substance abuse treatment, and family and social support systems.

Ensures accurate, timely, and appropriate documentation of interventions and outcomes in the designated record-keeping system.


Collaborates with the healthcare team, patients, families, and relevant community resources to ensure that oncology patients comprehend and actively engage in their care plans.

Continuously monitors and evaluates plans, options, and available services to effectively address patients' needs.


Qualifications:
Bachelor's degree in Nursing with a minimum of seven to ten years of clinical experience.

Oncology experience is essential.

Preferred experience in Case Management.

OCN certification is also preferred.

Licensure, Certifications, and Clearances:
Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification required based on AHA standards, including both didactic and skills demonstration components.

Registered Nurse (RN) licensure.

Act 34 clearance.

*Current licensure must be either in the state where the facility operates or, if applicable, a multistate license issued by a participating NLC state. Employees with an out-of-state NLC license who change residency to the state where the facility is located will have 60 days to apply for licensure in that state.
