Overview

Position Summary

The role of the Patient Nurse Navigator is to streamline and enhance the care experience for patients from the moment of diagnosis through their entire treatment journey. This position emphasizes patient advocacy, ensuring that individuals receive personalized support and access to necessary resources throughout their healthcare experience.

Key Responsibilities

  • Deliver tailored education and psychosocial support to newly diagnosed patients and their families, addressing specific needs related to their condition, while facilitating appropriate referrals for additional services.
  • Provide comprehensive information to both new and existing patients regarding their diagnosis, treatment options—including surgical and therapeutic interventions—financial considerations, community resources, and survivorship planning.
  • Act as a vital link between patients, families, and various departments within the healthcare system.
  • Oversee and coordinate new patient referrals, scheduling of necessary examinations and procedures, while identifying potential care gaps and ensuring continuity of care for patients transitioning back to their communities.
  • Initiate contact with every newly diagnosed patient promptly upon diagnosis.
  • Evaluate the emotional and social needs of new patients, identifying barriers to care such as insurance issues or transportation challenges, and connect them with appropriate resources.
  • Recognize and address trends that may hinder timely patient care, implementing service recovery strategies as necessary to enhance patient satisfaction.

Physical Requirements
Must be capable of lifting items weighing up to 25 pounds. The role may involve occasional stooping, kneeling, or crouching, and may require prolonged periods of sitting or standing.

Qualifications

A Bachelor of Science in Nursing (BSN) is required; a Master of Science in Nursing (MSN) is preferred. Current licensure as a Registered Nurse in North Carolina is mandatory. Certification in the relevant specialized field must be obtained within two years of assuming the position. Strong communication skills are essential, along with a preference for candidates with 4-5 years of experience in Acute Care Nursing.
